Requesting Sacramental Certificates
There will be times when you, your children or elderly parents may need a copy of the certificates of Baptism, Confirmation, or Marriage. The parish office can provide you a copy of the certificate in the Diocesan form as long as the sacrament was received either at St. Michael's Parish or St. Peter's Cathedral.
You may request a copy of the document by filling out an online form. Please click on the appropriate button below.
*Certificate copies are created for those who have already received the Sacrament.
PLEASE NOTE:
To obtain a baptismal certificate as documentation to support a citizenship application for individuals who have a distant relative born in Canada, please contact our diocesan Archivist, Deb Majer, at dbmajer@dol.ca.
